1. I consent to participate in this project, the details of which have been explained to me, and I have been provided with a written plain language statement to keep.
2. I understand that the purpose of this research is to explore and support the restoration of post-industrial scarred landscapes impacted by past quarrying and dumping activities, fostering community-led storytelling and co-designed strategies for resilient and adaptive future landscapes.
3. I understand that my participation in this project is for research purposes only.
4. I acknowledge that the possible effects of participating in this research project have been explained to my satisfaction.
5. In this project, I will be engaging in activities such as contributing to the digital platform (website) prototype through activities such as learning, sharing perspectives, insights and uploading materials of your lived experience and/or vision for the case study sites [☐ CERES / ☐ Darebin parklands / ☐ Hanson Landfill].
6. I understand that my participation will be contributing to a digital archive website for research and educational purpose only.
a. All material on the Site is made available on an “All Rights Reserved” basis, to be used only as permitted under the Copyright Act 1968 (Cth) or other relevant copyright law.
b. Notwithstanding clause 6.a, in some cases individual museums, galleries, and other collection sources have uploaded material to the Site under a more open licence (including under one of the licences referred to as “Creative Commons” licences). In such cases, we give no warranty or guarantee that the relevant collecting organisation or individual is entitled to grant the relevant licence, and if you are unclear as to the situation, you should check the position with the relevant collection source.
7. I understand that the output of my creative work during the digital platform prototype participation activities will be used for research purposes.
8. I will also honestly declare whether I am the original creator of the contribution to the named researchers. If I am not the original creator of the contribution, I will inform the named researchers about the original creator's details.
9. I understand that my participation is voluntary and that I am free to withdraw from this project anytime without explanation or prejudice and to withdraw any unprocessed data that I have provided.
10. I understand that the summary record of research data from this research will be retained as University Archives permanently.
11. I have been informed that the confidentiality of the information I provide will be safeguarded subject to any legal requirements; my data will be password protected and accessible only by the named researchers.
12. I understand that all personal data from this research will be securely stored at on a password secured University database that only the named researchers can gain access to. I understand that my de-identified data may be used for future research projects that are closely related to, or in the same general area as this research.
13. I understand that after I choose ‘YES’ in the survey question ‘‘I have read the Consent Form for digital participation and agree to allow the researcher to use the information I provide in this survey for research and educational purposes.’’, my contributed data will be retained by the named researchers and shared on the digital prototype.
